These Terms of Service ("Terms") govern your access to and use of the Forma platform ("Forma", "we", "us"). By creating an account or using Forma, you agree to these Terms. If you do not agree, please do not use Forma.
Forma is intended for adults aged 18 and over. By using Forma you confirm that you are at least 18 years old and able to enter into a binding agreement.
You agree to provide accurate information and to keep your login details secure. You are responsible for activity that occurs under your account, and should tell us promptly if you suspect unauthorised access.
Forma is a marketplace that helps members discover and connect with independent personal trainers. Trainers are independent professionals, not employees or agents of Forma. We do not provide coaching ourselves and are not responsible for the services trainers deliver.
Members pay the price set by their chosen trainer plus a Platform Service Fee shown at checkout. The rate is a flat 4%, with a minimum of 99c on digital items such as guides. The exact amount is always shown before you pay, which funds secure hosting, data protection, payment infrastructure and support. Trainers receive 100% of their set price. Forma takes no commission and makes no deduction from a trainer’s price; card processing and payout costs are met by Forma out of the Platform Service Fee. Payments are processed by Stripe. Subscriptions renew automatically until cancelled; you can cancel any time, effective at the end of the current period. Except where required by the Australian Consumer Law, fees paid are generally non-refundable.
If you offer coaching through Forma, you are responsible for holding any qualifications, registrations, and insurance your services require, for working within your professional scope of practice, and for complying with all applicable laws. You set your own prices and deliver your own services.
30-day rematch guarantee. If your first coach match isn’t the right fit, contact us within 30 days of starting and we’ll help you rematch with another coach at no extra platform cost. Amounts already paid for coaching are handled under the original coach’s stated policy — the guarantee covers rematching, not automatic refunds.
Information provided through Forma, including workouts and any nutrition or calorie and macro guidance, is general in nature and is not medical, clinical, or dietetic advice, nor a substitute for advice from a qualified health professional. Always consult your doctor before starting any exercise or nutrition program, and consult an Accredited Practising Dietitian (APD) for individualised nutrition needs. You participate at your own risk.
Payments are processed by our third-party provider, Stripe. Member subscriptions and one-off purchases are billed through Stripe. Members are charged the trainer’s set price plus a Platform Service Fee added at checkout — a flat 4%, with a minimum of 99c on digital items; this fee funds secure hosting and data protection, the operation of the Forma platform, payment infrastructure and support. The trainer receives 100% of their set price. Forma makes no deduction and takes no commission from the trainer’s rate. Wearable and health data (Forma Premium): where a member connects a third-party device or service, Forma accesses read-only activity, sleep and heart metrics solely to display them to the member and, only with the member’s explicit in-app consent, to their current coach. Health data is never sold or used for advertising; access tokens are stored server-side; disconnecting a provider revokes access and the member may delete synced data at any time. Trainers may use Forma free with one (1) active client; to take on more clients and unlock additional features a trainer must hold a paid Forma plan (Starter at A$29/month, Momentum at A$59/month, Practice at A$85/month, Studio at A$109/month, or Academy at A$189/month). Subscriptions renew automatically until cancelled. You can cancel future renewals at any time; unless required by law or stated otherwise, fees already paid are non-refundable. Prices are in Australian dollars and may include GST where applicable.
Forma invests time and money in finding members and introducing them to trainers. Where you connect with a member through Forma — any member you are matched with, introduced to, or who first contacts or subscribes to you via the platform (a “Forma-Sourced Client”) — the following apply for as long as you coach that person and, in any case, for a minimum of six (6) months from the date they first subscribe to you.
Pay through Forma only. All payments from a Forma-Sourced Client for your coaching must be made through Forma’s payment system. You must not invoice, bill, or take payment from a Forma-Sourced Client by any other means — including your own EFTPOS or card terminals, bank or account transfers, cash, or any third-party payment app or provider — and you must not arrange, encourage, or accept payment off the platform.
Client details. A Forma-Sourced Client’s contact and payment information is made available to you only once that member holds an active paid subscription with you on Forma. You may use those details solely to deliver coaching through the platform, and must not use them to move the member to any off-platform arrangement.
Minimum six-month term. You agree to keep a Forma-Sourced Client’s coaching payments on Forma for at least six (6) months from their first subscription. Cancelling, pausing, or shifting the arrangement off the platform within that period in order to avoid Forma’s fees is a breach of these Terms.
Liquidated damages. Circumventing Forma in any of the ways above deprives us of the platform fees we would otherwise have earned and the cost we incurred sourcing that member, which are genuinely difficult to calculate precisely. You and Forma therefore agree that a reasonable pre-estimate of that loss is A$750 for each Forma-Sourced Client affected, payable to Forma as liquidated damages (and not as a penalty) on breach. Forma may reduce or waive this amount at its discretion. This clause does not limit any other right or remedy available to Forma at law, including recovering unpaid platform fees.
You retain ownership of the content you upload (such as videos and workout information) and grant Forma a non-exclusive licence to host, process, and display that content for the purpose of operating the platform. You are responsible for your content and must have the rights to share it. We may review, moderate, or remove content that breaches these Terms or the law.
You agree not to misuse Forma, including by breaking the law, infringing others' rights, uploading harmful or misleading content, attempting to interfere with the platform's operation or security, or harassing other users.
All content, features, and functionality on the Forma platform—including but not limited to text, graphics, logos, user interfaces, underlying HTML/CSS, and software code—are the exclusive property of Forma and are protected by Australian and international copyright, trademark, and other intellectual property laws.
By using Forma, you agree that you will not:
We grant you a limited, non-exclusive, non-transferable license to access and use the platform strictly in accordance with these Terms. Any unauthorized use of our intellectual property immediately terminates this license.
Forma is provided "as is" and "as available". To the extent permitted by law, we make no warranties that the platform will be uninterrupted, error-free, or fit for a particular purpose. Nothing in these Terms excludes rights you have under the Australian Consumer Law that cannot lawfully be excluded.
To the extent permitted by law, Forma is not liable for any indirect, incidental, or consequential loss arising from your use of the platform or from services provided by trainers. Where our liability cannot be excluded, it is limited to the amount you paid to Forma in the three months before the claim.
You may stop using Forma at any time. We may suspend or terminate access if you breach these Terms or to protect the platform or its users.
These Terms are governed by the laws of Victoria, Australia, and you submit to the non-exclusive jurisdiction of its courts.
We may update these Terms from time to time; continued use of Forma means you accept the updated Terms. For any questions about these Terms, contact us at support@formapt.net.
You can close your account at any time from Settings → Danger zone. Closure takes effect after a 14-day notice period. During those 14 days your account works normally, any subscription continues, and final payments are collected. On the 14th day your account and its data are permanently deleted automatically — there is nothing further you need to do, and you do not need to contact us. You can cancel the closure at any time before that date.
If you are a coach, closing your account also cancels your clients’ subscriptions to you. We email your clients when you request closure, so they have notice to find another coach, and again when it takes effect. You remain responsible for coaching you have already been paid for during the notice period.
Forma has zero tolerance for objectionable content or abusive behaviour. You must not post content that is sexual, hateful, threatening or harassing, or that shares another person’s private information. Reviews, intro videos and transformation photos are screened automatically before they appear publicly, and anything reported is reviewed. Accounts that break this clause are removed without notice and without refund.
A negative review is not objectionable content. Members are entitled to their honest opinion of coaching they paid for, and we do not remove reviews for being critical, blunt or low-rated.
Where a coach offers a free trial, it is available once per member, per coach. To keep that fair, a limited record survives account deletion. The Privacy Policy sets out exactly what is kept, why, and for how long.

This policy explains how Forma collects, uses, and discloses your personal information. We handle personal information in line with the Australian Privacy Act 1988 (Cth) and the Australian Privacy Principles (APPs). Because we handle health information and are based in Victoria, we also handle it in line with the Health Records Act 2001 (Vic) and the Health Privacy Principles, and with equivalent health records laws in other states as we expand.
Because Forma is a fitness service, we collect health information — which is “sensitive information” under the Privacy Act and receives extra protection. This may include your weight and measurements, training logs, food-diary entries, sleep, heart rate, HRV and activity data (including from connected wearables), progress photos, and screening responses. We collect this only with your consent and only where reasonably necessary to provide the Service. You can withdraw consent at any time by disconnecting a source or contacting us. Forma never sells your health data and never uses it for advertising.
If you connect a service like Fitbit, Strava, Oura or Garmin, we access only the read-only data you authorise, to show it to you and — only if you explicitly turn on sharing — to your coach. You can disconnect any provider at any time, which revokes access, and you can delete the synced data.
We use your information to provide and operate Forma — including creating your account, matching members with trainers, displaying profiles and content, processing payments, moderating content for safety, and providing support and improvements.
We use trusted third parties to run Forma, and share only the information each needs to do its job:
Some of these providers are located outside Australia — Stripe, Mux, Google and Resend all process data in the United States — so using Forma involves disclosing your information overseas. We take reasonable steps to ensure overseas recipients handle your information consistently with the Australian Privacy Principles, and we remain accountable to you for how they handle it.
To help keep Forma safe, sample still frames from uploaded videos are sent to Google's Gemini AI for automated moderation. We also use Gemini to interpret member preferences for matchmaking and to generate general example meal ideas in the trainer nutrition tool. These features are not used to make decisions with legal or similarly significant effects without human review.
We take reasonable steps to protect your information, including technical and organisational measures such as encryption in transit and at rest, access controls, and restricting each coach to their own clients’ data. No method of transmission or storage is completely secure.
We keep your information while your account is active. Because health information is involved, we generally retain it for 7 years from your last session, in line with Australian health records requirements, after which we delete or de-identify it. You can ask us to delete your information sooner, and we will do so except where we are required by law to keep it.
You can access and update most of your information in the app, and you can ask us to correct or delete your personal information. Forma uses local storage on your device to remember your preferences and keep you signed in.
Forma is intended for adults aged 18 and over and is not directed at children.
We only send you marketing if you have opted in, and every marketing email includes a one-click unsubscribe link. We never use your health information for marketing. Opting out of marketing does not stop essential service emails such as booking confirmations and account notices.
If you think we have mishandled your personal information, contact us at support@formapt.net. We will acknowledge your complaint within 5 business days and aim to respond within 30 days. If you are not satisfied with our response, you can escalate to the Office of the Australian Information Commissioner (OAIC) at oaic.gov.au or 1300 363 992. Victorian residents may also complain to the Health Complaints Commissioner at hcc.vic.gov.au.
We may update this policy from time to time. For privacy questions, or to access, correct, or delete your information, contact us at support@formapt.net.
When you close your account we delete your data: your logs, photos, videos, measurements, check-ins, messages, coaching notes and profile. Records of bookings, subscriptions and payments are kept for seven years because tax and business law requires us to keep them. This happens automatically 14 days after you ask for it. We do not keep a copy for marketing and we do not sell it — not before, and not after.
One thing outlives deletion. When your account is deleted we store a one-way cryptographic hash (SHA-256) of your email address, alongside a list of which coaches you had already taken a free trial with. It exists for one reason: to stop somebody deleting and recreating an account to take the same coach’s free trial over and over, which takes money directly out of that coach’s pocket.
A one-way hash cannot be turned back into your email address. We keep no name, no user id, no messages, no photos and no other record of you. It cannot be used to identify you, to contact you, or to rebuild your account. It can answer exactly one question: has this address already had a trial with this coach?
This record is automatically deleted 12 months after your account closes. We rely on our legitimate interest in preventing fraud against the coaches on our platform (Australian Privacy Principles, and Article 6(1)(f) GDPR where that applies to you). If you want it removed sooner, email support@formapt.net.
Reviews, intro videos and transformation photos are screened by an automated system before they appear publicly, so objectionable content does not reach other people. The screening looks only at the content you submit — not at your logs, photos, messages or any other data — and it does not make decisions about your account. Anything reported is reviewed by a person.
